According to Connecticut General Statutes Sec. 38a-0472f, a party terminating a provider's participation agreement must provide 90-day written notice.
Additionally, any provider who receives or issues a termination notice must provide a list of UnitedHealthcare members who have been treated within the last 12 months. This list must be provided within 30 days of issuing or receiving a notice of termination.
