Course overview

The aim of the course is to provide tools on the various Healthcare Effectiveness Data and Information Set (HEDIS®) measures related to care given in a hospital and as patients transition home. It goes over hospital measures to help highlight the correlation and differences between each one.

When hospital measures are addressed as a whole and individually, it may help improve patient engagement, reduce readmissions, improve transitions of care, improve the patient experience and help members live healthier lives.

You’ll also find ways to help improve HEDIS® compliance, reduce organizational costs and ultimately improve Star Rating performance.

What are the hospital measures?

  • Hospitalization for Potentially Preventable Conditions (HPC)
  • Plan All Cause Readmission (PCR)
  • Follow up Within 7 Days from Emergency Department Visit for Multiple Chronic Conditions (FMC)
  • Transitions of Care (TRC)
    • Notification of Inpatient Admission (TRCRA)
    • Receipt of Discharge (TRCRD)
    • Patient Engagement After Inpatient Discharge (TRCPE)
    • Medication Reconciliation Post Discharge (TRCMRP)
