Minnesota Medicaid: New programs launching Jan. 1, 2023

UnitedHealthcare Community Plan of Minnesota has contracted with the Minnesota Department of Human Services and Centers for Medicare and Medicaid to provide Minnesota Senior Health Options and Minnesota Senior Care Plus and Special Needs BasicCare (SNBC).

Minnesota Senior Health Options is a Medicare Dual Eligible Special Needs Plan (DSNP) that integrates Medicare and Medicaid services, including acute and long-term care services for persons aged 65 and over who are eligible for both Medicare and Medicaid. Minnesota Senior Care Plus (MSC+), provides all health benefits through the Prepaid Medical Assistance Medical Care program to eligible enrollees including Home and Community-Based Waiver Services (HCBS).

The Special Needs BasicCare (SNBC) program serves people with disabilities ages 18-64. SNBC creates an alternative health care and support services delivery system for people with disabilities who are eligible for Medicaid or have dual eligibility for Medicaid and Medicare. The SNBC program is a public health benefits program intended to provide enrollees with access to cost-effective health care options. UnitedHealthcare will offer both a stand-alone Medicaid-only SNBC plan and an integrated SNBC DSNP.

Enroll to provide services

You must be enrolled as a Minnesota Health Care Programs (MHCP) provider to provide Medicaid services to Minnesota Medicaid members. If you are not enrolled, you may request enrollment by using the Minnesota Provider Screening and Enrollment (MPSE) portal or by submitting the application materials required by fax at 651-431-7462.

How it works

Care coordinators will work with you to help put together all your physical, behavioral health and long-term services and supports into a personal care plan to deliver care that meets your patients’ needs.

Provider training and resources
