Arizona Medicaid: Prior authorization and notification updates

Occupational and physical therapy changes starting Feb. 1, 2023

Beginning Feb. 1, 2023, we are updating the current prior authorization process for occupational therapy and physical therapy to align with existing policies governing speech therapy services. After the Feb. 1 effective date, we will require authorization after an initial evaluation and before an initial therapy visit.

This change will impact UnitedHealthcare Community Plan of Arizona Health Care Cost Containment System (AHCCCS) Complete Care members.

Prior authorization will be required for members under the age of 21 for ongoing physical, occupational and speech therapy services. Please submit prior authorization requests for these services with dates of service on or after Feb. 1, 2023, through one of the following options:

For the full list of therapy codes, reference the current prior authorization plan requirements for AHCCCS and Medicaid long-term care plans. The lists will be updated prior to the effective date.
