Maryland program for patients with complex needs

The state of Maryland offers an alternative case management program for Medicaid HealthChoice members with specified rare and expensive conditions.

The Rare and Expensive Case Management (REM) program assigns a case manager who provides care coordination, home visits and other case management services to help members with complex or special health care needs.

Referrals and eligibility

Consider referring eligible patients to the REM program. Referrals may also be submitted by the member or member's family — enrollment is voluntary. When your patient is enrolled in the REM program, you will be notified by letter.

Additionally, when members are enrolled in the REM program, they are disenrolled from UnitedHealthcare Community Plan.
