Capitation and/or delegation supplement - 2022 UnitedHealthcare Administrative Guide

This supplement is for participating physicians, health care providers, facilities and capitated ancillary providers as well as delegated accountable care organizations (ACOs) for certain UnitedHealthcare products. It applies to all benefit plans for members who:

1. Have been assigned to or have chosen a health care provider who receives a capitation payment from us for that member, and
2. Are covered under an applicable benefit plan under UnitedHealthcare.
