New Disease Management Program in New Jersey for End-Stage Renal Disease

If you determine that one of your UnitedHealthcare Dual Complete® One members is diagnosed with chronic kidney disease, such as end-stage renal disease (ESRD) or critical renal failure (CRF), UnitedHealthcare is offering a supplemental disease management program to support the member – at no additional cost to the member.

The program, provided by Kidney Resource Services (KRS) through OptumCare, provides access to KRS registered nurses to help people live with kidney disease and provide helpful information on:

  • The member’s dialysis
  • Nutritional needs and medications
  • Fluid and weight management

They’ll work with the member’s health care team to help coordinate their care and track any related health issues all while supporting the relationship and treatment plan set up with the patient’s care provider.

The program is voluntary, and members can enroll or disenroll at any time. Share the KRS website and phone number, 866-561-7518, with any UnitedHealthcare Dual Complete One member who you feel may benefit from this program.
