Practice Matters: Family Visiting program

The Family Visiting program (FVP) is designed to support families with risk factors that could impede the development of strong family relationships.

How it works

Evidence-Informed Organizations (EIOs) and Evidence-Based Organizations (EBOs) provide home visiting services, assessments and referrals for ongoing services for first-time and high-risk mothers, parents and infants with known and suspected risk factors, and pregnant women or family expressing interest in enrolling in FVP.

In addition to contracted EIOs/EBOs, UnitedHealthcare Community Plan of Pennsylvania Community Health Workers (CHWs) also provide home-visiting services as we work to increase the number of contracted EIOs/EBOs in additional counties. EIO/EBOs can also provide additional visits for families who need ongoing face-to-face support.

If a mother loses eligibility after delivery and the infant is still enrolled in FVP, we’ll continue services under the infant’s name and case record until the child reaches age 18 months.
