All re-credentialing documents are required to be submitted 8 weeks before your re-credentialing date

September 2021

You are required to recredential your qualifications as a hearing professional with UnitedHealthcare Hearing every 3 years. As part of the process, you will begin receiving notifications 6 months before your recredentialing is due. All documentation must be submitted 8 weeks before to stay on track with the recredentialing process and avoid major credentialing issues.

Required documents include:

  • Updated license
  • Liability insurance certificate
  • Updated W-9
  • Signed provider agreement
  • Updated provider information sheet

The 8-week deadline is firm and needed to process all documents through Gemini. If you do not complete the submission of required recredentialing documents by your due date:

You can upload your documents easily using the Provider Portal. Our new, self-paced training course will show you the steps to take, what documents to send and how to submit them online.
