You are required to recredential your qualifications as a hearing professional with UnitedHealthcare Hearing every 3 years. As part of the process, you will begin receiving notifications 6 months before your recredentialing is due. All documentation must be submitted 8 weeks before to stay on track with the recredentialing process and avoid major credentialing issues.
Required documents include:
- Updated license
- Liability insurance certificate
- Updated W-9
- Signed provider agreement
- Updated provider information sheet
The 8-week deadline is firm and needed to process all documents through Gemini. If you do not complete the submission of required recredentialing documents by your due date:
- You will need to start the credentialing process over again
- Patients will be reassigned to other practices
- Your access to the UnitedHealthcare Hearing Provider Portal will be interrupted
