Prior authorization and site of service updates

Starting Feb. 1, 2022, we’re updating the prior authorization requirements and site of service medical necessity reviews for certain surgical procedures. These updates apply to:

• Commercial benefit plans, including Oxford, Neighborhood Health Partnership, UHOne, All Savers, UnitedHealthcare of the Mid-Atlantic, Inc., MAHP Life and Health Insurance Company and UnitedHealthcare of the River Valley
• Individual Exchange plans

What’s changing — effective Feb. 1, 2022

Certain CPT^® or HCPCS codes will no longer require prior authorization, and we will no longer conduct site of service medical necessity reviews for these codes.
