New York Medicaid: Complete the obstetrical risk assessment form to earn an incentive
Network care providers can now earn a $25 incentive for completing the obstetrical risk assessment form (OBRAF) within 5 days of a UnitedHealthcare Community Plan member’s first prenatal care visit.
We’ll pay these incentives on a quarterly basis.
Completing the OBRAF
You can complete the OBRAF using the Care Conductor and Notification of Pregnancy tool in the UnitedHealthcare Provider Portal. Here’s how:
- Sign in using your One Healthcare ID. New users can register at UHCprovider.com/newuser.
- Select Care Conductor and Notification of Pregnancy within the Clinical and Pharmacy dropdown menu
