Ohio: Access resources that may help change the lives of your diabetes patients

The number of people diagnosed with diabetes has continued to grow over the years. To find ways to help support your care for these patients, UnitedHealthcare Community Plan of Ohio has collaborated with several Medicaid managed care organizations, the Ohio Department of Medicaid and the Ohio Association of Health Plans. Together, we created a new online tool, Diabetes Resources for Providers. This resource is designed to guide you in your efforts to improve the lives of your patients who have been diagnosed with diabetes.

The Diabetes Resources for Providers page is filled with news, videos, blogs and other content on a statewide basis. Plus, you can link directly to helpful information that you can pass along to your patients on monitoring and self-managing their diabetic care.

Key features of the site

The new resource page includes:

  • An introduction to diabetes work throughout Ohio
  • Care coordination approach and roles
  • Information on health care benefits
  • Resources for diabetes self-management education (DSME) and medical nutrition therapy (MNT)
  • Additional articles and information related to diabetes in Ohio

Working together to help improve outcomes

This robust resource page is one way we’ve aligned efforts and resources to help advance the health outcomes in the Medicaid diabetic population. We’ve also standardized quantity limits to diabetic supplies and collaborated with vendors and pharmacists to make them available at the corner drugstore. By working together, we're making it easier for members of Medicaid plans to get the supplies and support needed to better manage their chronic condition.
