Prior authorization for preimplantation genetic testing and fertility preservation

Effective July 1, 2021, for new and renewing businesses with 51+ employees with 2021 certificate of coverage (COC), preimplantation genetic testing (PGT) and fertility preservation for iatrogenic infertility will be covered as a standard benefit for eligible UnitedHealthcare commercial plan members. This coverage will be rolled out Jan. 1, 2022 for commercial small business plan members on new or renewing plans.

Preimplantation genetic testing

PGT is covered when a patient or their partner is a carrier of a specific genetic disorder. This benefit focuses on the types of genetic testing that can diagnose specific inherited conditions caused by a single gene (PGT-M) or by structural changes of a parent’s chromosome (PGT-SR) that could result in significant health problems or severe disability.

You’ll need to complete the prior authorization process and provide any applicable medical documentation.

Fertility preservation

We will cover fertility preservation for medical reasons that cause irreversible infertility, such as chemotherapy, radiation treatment and bilateral oophorectomy due to cancer.
