The TennCare Regulatory Appendix has recently been updated. The Appendix is routinely appended to our TennCare provider agreements.
What this means for you
If you object to this Appendix or no longer wish to participate in the UnitedHealthcare Community Plan of Tennessee network, you must provide written notice on official letterhead to UnitedHealthcare Community Plan within 30 days from the date of this notice. Notice may be sent by first-class mail to the following address:
