PCSP and FA available on Provider Portal

Person-Centered Support Plans (PCSP) and Functional Assessments (FA) are available for individuals enrolled in the Tennessee Long-Term Services & Supports (LTSS) program. These assessments provide an in-depth evaluation of the needs and services provided to individuals enrolled in the LTSS program.  View and download for individuals­­ who are part of TennCare’s Long-Term Services & Supports programs (CHOICES and Employment and Community First CHOICES).

These plans and assessments communicate patient values to guide clinical decisions. You can gain a better understanding of individual needs, wants and choices for care in addition to services provided in your patient’s place of residence.

Person-Centered Support Plan (PCSP)

  • Individually identified health and wellness goals
  • LTSS and natural supports to support independent living in a community or community-based residential alternative (CBRA) living setting

LTSS Functional Assessment (FA)

  • A functional evaluation tool that captures the individual’s ability to perform activities of daily living (ADL) and instrumental activities of daily living (IADL)
  • Provides a functional score for determination of services to support the person in their home
