Maryland Medicaid: Apply for AHEAD Primary Care Programs by May 22

The Maryland Department of Health (MDH) has launched the AHEAD Primary Care Programs Application for Performance Year (PY) 2027. This application is for primary care practice organizations that are not currently participating in the Medicaid Advanced Primary Care Program.

As part of this application, you’ll also have an opportunity to indicate your interest in PC AHEAD or MDPCP-AHEAD, 2 Medicare advanced primary care programs available in Maryland. 

Please submit your application before Friday, May 22 at 5 p.m

General eligibility requirements

Primary care practice organizations must meet the following criteria:

• A minimum of 250 assigned HealthChoice members across all managed care organizations (MCOs)
• Utilization of an electronic health record (EHR)
• Ability to report electronic Clinical Quality Measures (eCQMs)
• Connection to Chesapeake Regional Information System for our Patients (CRISP) or willingness to connect before the end of 2026
