Florida Medicaid: Increased OBRAF incentive

Earn $100 incentive per OBRAF submitted in 2024

We are offering an increased incentive to Florida providers who complete an Obstetrical Risk Assessment Form (OBRAF) for pregnant patients who are UnitedHealthcare Community Plan members.

How the form benefits your patient

Using the information gathered from the OBRAF, we’ll enroll eligible patients in our Healthy First Steps® program and provide them with information and touchpoints to help:

  • Reinforce your care plan
  • Encourage healthy behaviors that may help improve maternity health outcomes
  • Identify high-risk patients and assign a nurse coach to help with prenatal and postpartum care
  • Give access to social support resources to address food, housing and safety needs
  • Meet HEDIS® standards

How to submit the form

  • You must submit the form within 10 days of your patient’s first prenatal visit to receive the incentive
  • You can submit the form online or by fax
  • You don’t need to submit a claim to receive the incentive

Online (preferred method):

  1. Visit the UnitedHealthcare Provider Portal and sign in
  2. Select Clinical and Pharmacy and then Care Conductor and Notification of Pregnancy
  3. Enter the required information and complete the OBRAF on the last page

Fax

  1. Visit the FL Provider Forms and Resources page
  2. Select Obstetrical Risk Assessment Form (OBRAF)
  3. Fax the completed form to us at 877-353-6913
