Arizona Medicaid: Updated requirements for anthropometric and biochemical data

Starting Oct. 1, 2023, the Arizona Special Supplemental Nutrition Program for Women, Infants and Children (WIC) program will require you to complete anthropometric and biochemical documentation for WIC participants who are your patients. Requirements include height/length, weight and hemoglobin/hematocrit data.

Acceptable forms of documentation

  • Health Data Form
  • Referral and Physical Presence Exemption Form
  • Formula and Food Request Form

Additional acceptable documentation includes:

  • Health care professional letterhead with anthropometric/biochemical data
  • Health care professional prescription form
  • Printed medical record with measurement dates and the health care professional’s name
  • Providing verbal medical data to WIC staff

Next steps

After you complete the form, hand it to the patient, fax it to your local WIC office or upload it to your patient portal. If you give it to the patient, the patient will need to submit it to their local WIC office.
