New genetic and molecular prior authorization codes

To comply with the updates made by the American Medical Association, effective July 1, 2026, we’ll add new procedure codes to our prior authorization requirements for genetic and molecular testing. This change applies to certain UnitedHealthcare commercial plans and UnitedHealthcare Individual Exchange Plans. 

Affected plans

This change affects the following plans in all states:

• Neighborhood Health Partnership, Inc.
• UnitedHealthcare Level Funded
• UnitedHealthcare Oxford 
• UnitedHealthcare of the River Valley
• UnitedHealthcare Individual Exchange Plans

New codes

In addition to the current in-scope procedure codes, we’ll add the following codes to the existing prior authorization/advance notification requirements:

How to submit and manage prior authorizations

• Sign in to the UnitedHealthcare Provider Portal with your One Healthcare ID 
  • If you don’t have a One Healthcare ID, register now
• In the menu, select Prior Authorizations
• In the Prior Authorization & Notification tool, scroll down to “Create a new notification or prior authorization request” and click Create New Submissions
• Enter the required information and submit
