Medicaid: Review genetic and molecular code changes

To comply with the American Medical Association code changes, effective July 1, 2026, we’ll be updating prior authorization or advance notification requirements for the following genetic and molecular procedure codes. Please review the state-specific lists to determine applicable prior authorization or advance notification requirement additions or removals. 

Affected plans

Effective July 1, 2026, prior authorizations for these genetic and molecular testing codes will apply to the following UnitedHealthcare Community Plans:

  • Florida
  • Hawaii
  • Kansas CHIP
  • Kansas 
  • Kentucky
  • Maryland
  • Michigan
  • Missouri
  • Nebraska
  • New Jersey
  • New Mexico
  • New York
  • Pennsylvania CHIP

  • Washington
  • Wisconsin

How to submit and manage prior authorizations

  • Sign in to the UnitedHealthcare Provider Portal with your One Healthcare ID
  • From the left-hand tabs, select Prior Authorizations & Notifications
  • In the Prior Authorization and Notification tool, select Create a new notification or prior authorization request and click Create New Submissions
  • Enter the required information and submit
