Michigan: MCT2D CGM prior auth bypass pilot ending Jan. 1

The Michigan Collaborative for Type 2 Diabetes (MCT2D) prior authorization bypass pilot program for continuous glucose monitors (CGM) was designed as a short-term program and will conclude on Jan. 1, 2027. This change applies to UnitedHealthcare commercial members.

When the pilot program ends on Jan. 1, 2027, members will need to meet our standard CGM coverage criteria and you may need to complete the prior authorization process before coverage can begin.

Coverage details through Dec. 31, 2026

For health care professionals participating in the MCT2D program up until Dec. 31, 2026, the prior authorization bypass process will remain in place for CGM prescriptions for members with type 2 diabetes.

Members who are prescribed a CGM under the current MCT2D exception on or before Dec. 31, 2026, will continue to be eligible for CGM coverage based solely on a diagnosis of type 2 diabetes and will not be required to complete prior authorization for 12 months, or Dec. 31, 2027, whichever comes first.
