Ohio Medicaid enrollment requirement

You must enroll with Ohio Medicaid if you participate in a Medicaid Managed Care Organization (MCO) provider network, as required by federal regulations.

As of Jan. 1, 2026, if you aren't properly enrolled, you’ll be removed from the UnitedHealthcare Community Vision Network / March Vision Network, and your claims will be denied.

What you need to do

Visit the Ohio Department of Medicaid website to enroll or confirm your enrollment.

Make sure these claim details match your Ohio Medicaid enrollment record when submitting a claim:

• Billing NPI
• Rendering NPI
• Date-of-service (must fall within your Medicaid agreement and specialty effective dates)
• If Rendering NPI is part of a group:
  • The Group ID must match the verified Billing NPI’s Medicaid ID
  • The date-of-service must be within the group’s effective dates
