Vision benefit plan terminations for Molina Healthcare plans

August 2026

UnitedHealthcare Community Vision Network/March Vision Network will no longer manage vision benefits for Molina Healthcare plans in several states beginning this fall.

Providers may continue to treat affected members and receive reimbursement for eligible services through the applicable termination date listed below. After these dates, vision benefits for these plans will no longer be administered through UnitedHealthcare Community Vision Network/March Vision Network.

Affected plans and termination dates

Effective Sept. 1, 2026

Providers may continue to provide covered services and submit claims for reimbursement through Aug. 31, 2026, for:

  • Kentucky: Passport by Molina Healthcare
  • Nebraska: Molina Healthcare of Nebraska
  • New Mexico: Molina Healthcare of New Mexico
  • Ohio: Molina Healthcare of Ohio

Effective Oct. 1, 2026

Providers may continue to provide covered services and submit claims for reimbursement through Sept. 30, 2026, for:

  • Mississippi: Molina Healthcare of Mississippi

What this means

If your practice serves members enrolled in any of the affected plans, be sure to verify patient eligibility and benefit administration before scheduling appointments for dates of service after the termination effective date. Providers should also communicate any necessary changes to office staff responsible for scheduling, eligibility verification and claims submission.

Plans not affected

These changes apply only to the Molina Healthcare plans listed above.

The following plans will continue to be administered through UnitedHealthcare Community Vision Network/March Vision Network:

  • Kentucky: UnitedHealthcare Community Plan and UnitedHealthcare Dual Complete
  • Nebraska: Other UnitedHealthcare vision benefit plans in the state are not affected by this change
  • New Mexico: UnitedHealthcare Community Plan of New Mexico
  • Ohio: UnitedHealthcare Community Plan and UnitedHealthcare Dual Complete
