Many benefits change at the beginning of the year. In addition to the following benefit updates, there are new plan designs for our Medicare plans, both non-D-SNP and D-SNP, with updates in allowance and frequency. It’s important for providers to verify eligibility and benefits before rendering services to each patient. The following state list has updates for one or more of the benefit plans in each state, effective Jan. 1, 2025.
