Nebraska: External cause of injury codes

When claims for a UnitedHealthcare Community Plan member include a trauma diagnosis code, you must include an appropriate external cause of injury (ECI) diagnosis code in box 72 of the UB-04 form.

ECI code requirements

Claims with a diagnosis code that falls within these ICD-10 trauma diagnosis code ranges must include an ECI diagnosis code that begins with V, W, X or Y in box 72.

This applies to inpatient, outpatient and home health claims billed on a UB-04 form using an ICD-10 trauma diagnosis code within the following ranges:

  • K062–K062999
  • K0811–K081199
  • K08419–K084199
  • M991–M991999
  • S00–S999999
  • T07–T349999
  • Z04–Z043999

Failure to include an appropriate ECI diagnosis code may lead to claim denials or recoupments.
