Person-Centered Support Plans (PCSP) and Functional Assessments (FA) are available for individuals enrolled in the Tennessee Long-Term Services & Supports (LTSS) program. These assessments provide an in-depth evaluation of the needs and services provided to individuals enrolled in the LTSS program. View and download for individuals who are part of TennCare’s LTTS programs (CHOICES and Employment and Community First CHOICES).
These plans and assessments communicate patient values to guide clinical decisions. You can gain a better understanding of individual needs, wants and choices for care in addition to services provided in your patient’s place of residence.
Person-Centered Support Plan (PCSP)
- Individually identified health and wellness goals
- LTSS and natural supports to support independent living in a community or community-based residential alternative (CBRA) living setting
LTSS Functional Assessment (FA)
- A functional evaluation tool that captures the individual’s ability to perform activities of daily living (ADL) and instrumental activities of daily living (IADL)
- Provides a functional score for determination of services to support the person in their home
