This information applies only to clinics, facilities/organizations and ancillary care providers located within the state of California who would like to contract with UnitedHealthcare.
Some services require credentialing. Credentialing is the process of reviewing and verifying your professional credentials to assure the UnitedHealthcare Network consists of quality health care professionals who meet defined criteria and standards.
If a network is listed as “closed”, it means we are not accepting new providers at this time. You should check back frequently for updated network information.

Credentialing is not required for ambulance and transportation companies, however, credentialing for transportation services may be required under an HCBS/LTSS (Home & Community-Based Services /Long Term Services and Support) contract.
We require that ambulance and transportation companies submit a Letter of Interest before a contract is made.
Step 1: Letter of Interest
The Letter of Interest needs to include the following information:

Step 2: Contracts
After your Letter of Interest has been received and accepted, the contracting process will begin.
Contracts will not be made until a care provider or facility has submitted a completed Letter of Interest.
Ambulatory Surgery Centers, Congregate Living Facilities, Long-Term Acute Care Centers and Rehab Hospital care providers must be credentialed in order to participate in the UnitedHealthcare network.
The credentialing application process takes up to 25 calendar days for review and approval.
Once your credentialing is approved, your network management contact will help you begin the contracting process.
Step 1: Begin the Credentialing Process
Send us an email request at networkhelp@uhc.com and include the following information: care provider full name, tax ID, service address location(s), description of the type of facility (taxonomy).
Step 2: Credentialing Approval or Denial
We will notify you by U.S. Mail on whether your credentialing application has been approved or denied.
Step 3: Letter of Interest
Submit a Letter of Interest that clearly indicates that your facility has completed credentialing and has been approved. The Letter of Interest also needs to include the following information:

Step 4: Contracts

The network for Audiology is currently closed. We are not accepting new Audiology care providers at this time.
The network of Hearing Aid providers and all hearing aid services are provided through UnitedHealthcare Hearing.

The Diagnostic Radiology network is closed only in Los Angeles County.
Credentialing is not required for Diagnostic Radiology facilities, however, facilities must be Medicare Certified, accredited with the American College of Radiology or the Intersocietal Accreditation Commission, in order to participate in our network.
We do require that Diagnostic Radiology facilities submit a Letter of Interest before a contract can be issued.

We require facilities that would like to join the network for Pathology Services submit both a Letter of Interest and a Lab Inquiry Questionnaire.
