Effective April 1, 2021, prior authorization for certain outpatient radiation therapies will be required for UnitedHealthcare commercial members, including:
Rhode Island and Oxford Health Plan members will be excluded.

Effective April 1, 2021, prior authorization will be required for the following radiation therapy services, in addition to intensity-modulated radiation therapy and proton beam therapy:
Intensity-modulated radiation therapy (IMRT) and proton beam therapy (PBT)
Image-guided radiation therapy (IGRT), special services, Y90/SIRT or SBRT, including SRS:
To submit an online request for prior authorization, sign in to access the Prior Authorization and Notification tool. Select the “Radiology, Cardiology, Oncology and Radiation Therapy” box. After selecting “Commercial” as the product type, you will be directed to another website to process the authorization requests.
