Effective Jan. 1, 2022, UnitedHealthcare Connected® for One Care (Medicare-Medicaid plan) will be available to individuals in Massachusetts who are eligible for both Medicare and Medicaid.
This UnitedHealthcare Community Plan of Massachusetts plan provides dual-eligible members with a customized care plan and a single point of contact for all Medicare and MassHealth services. MassHealth has more information on member eligibility and overall plan benefits.
UnitedHealthcare Connected
When you see a UnitedHealthcare Connected member, you’ll work with a UnitedHealthcare care coordinator on the member’s care plan.
Verifying member eligibility
UnitedHealthcare Connected will be available for eligible individuals living in these counties: Bristol, Essex (partial), Franklin, Hampden, Hampshire, Middlesex, Plymouth, Suffolk and Worcester.
After Jan. 1, you’ll be able to verify the member’s enrollment through the UnitedHealthcare Provider Portal Eligibility and Benefits tool.
