Starting Oct. 1, 2022, we’re adding additional codes to the prior authorization list for UnitedHealthcare commercial plans, including:

The following Cosmetic & Reconstructive codes will be added to the prior authorization list effective Oct. 1, 2022.

Additionally, CPT code 64450 which was scheduled for a Nov. 1, 2022 implementation, will not deploy as announced.
