Hawaii Medicaid: Chemotherapy prior authorization update
Effective Jan. 1, 2023, the UnitedHealthcare Community Plan of Hawaii will be added to the existing prior authorization requirement for chemotherapy drugs received in an outpatient setting. Health care professionals must submit a prior authorization request before dispensing chemotherapy drugs in an outpatient setting to Community Plan of Hawaii members.
We will make determinations for prior authorization requests based on UnitedHealthcare clinical policy requirements for coverage.
How to submit prior authorization
