Last modified Oct. 30, 2023
Update: Contact and questions section updated to reflect new chat services

Health care professionals in The New York State Empire Plan can now use CAQH to verify, update and attest to their demographic data.

Keeping your practice information up-to-date helps members find you.

How to verify and attest
There are 4 ways to verify and attest to your demographic information accuracy.

  • CAQH (New option)
  • My Practice Profile (MPP) on the UnitedHealthcare Provider Portal
    • To access MPP, go to UHCprovider.com and select Sign In in the upper right corner
      • Sign in with your One Healthcare ID and password
      • If you need to set up a One Healthcare ID, visit New User & User Access
    • If you have trouble accessing the portal or MPP, support is available 7 a.m.–7 p.m. CT, Monday – Friday via chat. Sign in with your One Healthcare ID to chat with an advocate. Support is also available by calling 866-842-3278, option 1.
    • See our My Practice Profile Interactive Guide to learn more about MPP
  • Mail: Empire Plan Network Management
    UnitedHealthcare
    PO Box 2300
    Kingston, NY 12402-2300
  • Fax: 844-897-5439

Reminder – 90-day attestation requirements
You are now required to review, update and attest to your demographic data accuracy every 90 days, as outlined in the UnitedHealthcare Administrative Guide and required by the Consolidated Appropriations Act (CAA). Please see the January 2022 announcement for more details.

Please refer to your Participation Agreement for additional contractual notice requirements that may separately apply.

In most cases, you’ll be notified when it’s time to review your demographic information, based on how you’ve previously verified your data.
