Arizona: Suspect Medicaid fraud? Learn how to report it

The UnitedHealthcare Community Plan Fraud, Waste and Abuse Program focuses on prevention, detection and investigation of false and abusive acts committed by care providers and/or plan members.

What to do if you suspect fraud, waste or abuse

You’re required by law to report suspected incidents of fraud, waste or abuse committed by a plan member or care provider. The Arizona Health Care Cost Containment System (AHCCCS) requires that you report such incidents within 10 days of discovery.

How to report fraud, waste and abuse

There are several ways to report suspected incidents:

  • Online: Go to azahcccs.gov > Report Fraud
