New clinical pharmacist-led medication optimization program

We’re pleased to introduce our Comprehensive Medication Management (CMM) program, a partnership with Genoa Healthcare. The program offers a patient-centered, interactive review of current medications that may include prescription and over-the-counter medications, as well as herbal supplements.

This pharmacist-led program evaluates medication therapy for indication, effectiveness, safety and the patient’s ability to take their medications as prescribed. We use data obtained through pharmacy claims, medical claims and self-reporting.

The program is available for Community Plans, Medicare Advantage plans and Commercial plans.

How it works

  1. Selection: Individuals are identified for program inclusion through clinical algorithms.  
  2. Opt-in and release of information: A team member will help explain the program to the individual and offer them the opportunity to participate. Those who enroll in the program agree to engage with the clinical pharmacist. We’ll then enroll them for up to 12 months, as clinically indicated. We’ll also ask these individuals for their consent to share information with their treating health care professionals and health plan.
  3. Outreach to you: We’ll share this health information and applicable recommendations with you to help support their overall care coordination.
  4. Your action: You may receive notification via fax. The notification will include a list of the individual’s existing prescribed medications and we’ll recommend that you take action on 1 or more medications where there may be an opportunity for therapy optimization. There are 3 steps:

a. Review and respond to recommendations
b. Sign and return completed form within 14 days (optional)
c. Communicate all prescription changes with the individual and to their dispensing pharmacy (if applicable)
