Colorado: New Provider for Medicare Advantage HMO

Beginning Oct. 1, 2021, Preferred Homecare is the new capitated provider for the Medicare Advantage HMO members in Colorado.

You may receive a call from Preferred Homecare requesting an updated/new prescription for the equipment your patients’ are currently receiving. 

To ensure uninterrupted care, please promptly return signed orders for the equipment allowing Preferred Homecare to replace the equipment.

• Preferred Homecare will began taking new referrals for vents, oxygen, and trach on Sep. 1, 2021.
• Preferred Homecare will take all other new DME orders beginning Oct. 1, 2021
