In-home palliative care for Medicare Advantage members

We’re working with Aspire Health and Prospero Health to provide UnitedHealthcare® Medicare Advantage members with medical care and support at home for a chronic or long-term illness.

Medicare Advantage palliative care vendors

Both Aspire and Prospero are available to eligible members nationwide, however, the vendor serving the member depends on where the member lives. Aspire and Prospero will proactively identify and contact eligible members in their respective geographical service areas.

Referrals and what this means for you

We want to help ensure your patients who are UnitedHealthcare Medicare Advantage and Dual Special Needs Plan (DSNP) members have access to in-home palliative care if medically necessary.

When you determine that an eligible member may benefit from palliative care, you’re encouraged to refer them. You’ll start the referral process by contacting Aspire and/or Prospero at the contact information provided at the bottom of this memo. The vendor will be able to tell you whether or not they service your county.

Once you identify the appropriate vendor, you’ll fill out and submit the referral form. The vendor will enroll the member if the member qualifies. If the member doesn’t qualify, the vendor helps the member connect with more appropriate resources and will notify the member’s primary care and specialty health care team.

Member eligibility

Each vendor determines eligibility and reviews for medical necessity. Qualified members are usually all the following:

  • A UnitedHealthcare Medicare Advantage member (including DSNP)
  • Appears to be in the last 12 months of life
  • Has multiple medical chronic conditions or a recent inpatient or skilled nursing facility admission
