Arizona Medicaid: Updated requirements for anthropometric and biochemical data
Starting Oct. 1, 2023, the Arizona Special Supplemental Nutrition Program for Women, Infants and Children (WIC) program will require you to complete anthropometric and biochemical documentation for WIC participants who are your patients. Requirements include height/length, weight and hemoglobin/hematocrit data.
Health care professional letterhead with anthropometric/biochemical data
Health care professional prescription form
Printed medical record with measurement dates and the health care professional’s name
Providing verbal medical data to WIC staff
After you complete the form, hand it to the patient, fax it to your local WIC office or upload it to your patient portal. If you give it to the patient, the patient will need to submit it to their local WIC office.
