California: Verify your data to remain in our provider directories

Update: Updated data requirements and adjusted CAQH Portal name/link
As part of your contract and California Senate Bill 137, you must review and update your demographic data for accuracy every quarter. Demographic data includes office location, phone, hours, etc. If your data is inaccurate, we must remove your information from the provider directories.
Failure to update your directory information may result in a delay of payment or reimbursement of a claim under California Health & Safety Code Section 1367.27(p).
How to verify your demographic data
- CAQH Provider Data Portal
- Existing users (individual practitioners and groups): Sign in to your CAQH Provider Data Portal account
- New users: Go to CAQH Provider Data Portal to create a new, secure account for individual practitioners and groups
- Additional information on CAQH data management available online
- My Practice Profile
- Roster management: Contact your roster manager directly to attest, review and update your information
