Massachusetts UnitedHealthcare Connected® for One Care

Starting Jan. 1, 2022, UnitedHealthcare Connected for One Care plan provides dual-eligible members (ages 21 - 64 with disabilities) with a customized care plan and a single point of contact for all Medicare and MassHealth services. MassHealth has more information on member eligibility and overall plan benefits.

Eligible members

After Jan. 1, 2022, you’ll be able to verify the member’s enrollment through the UnitedHealthcare Provider Portal Eligibility and Benefits tool.

UnitedHealthcare Connected will be available for eligible individuals living in these counties: Bristol, Essex (partial), Franklin, Hampden, Hampshire, Middlesex, Plymouth, Suffolk and Worcester.

Members must be eligible for MassHealth Standard or MassHealth CommonHealth, enrolled in Medicare Parts A and B and eligible for Medicare Part D, and are without other comprehensive public or private insurance. Members can include adults with the following conditions:

  • Disabilities with multiple chronic illnesses or functional or cognitive limitations
  • Homeless members with disabilities
  • Intellectual or developmental disabilities
  • Physical disabilities
  • Serious mental illness
  • Substance use disorders
