Tennessee Medicaid: New ASH claim billing requirements

Effective July 1, 2023, UnitedHealthcare Community Plan of Tennessee will require medical records on all abortion, sterilization and hysterectomy (ASH) claims billed with the below CPT® codes, in addition to the completed federal ASH form, if applicable.

The applicable medical record is the minimum requirement if your claim contains at least 1 of the following CPT codes:
