Avoid directory removal — verify and update your demographic data

Since July 1, 2022, we’ve required you to attest to your provider data. We may remove unattested data from the UnitedHealthcare Provider Directory if we can’t verify your information. Removal is temporary until you complete the attestation process.

This applies only to health care professionals who are contracted in UnitedHealthcare commercial plans and UnitedHealthcare Individual Exchange plans.

90-day verification requirement

As announced in the January 2022 Network News, the Consolidation Appropriations Act requires network health care professionals to verify the accuracy of demographic information every 90 days. Your Participation Agreement may also have additional contractual notice requirements that may separately apply.

We understand the 90-day verification requirement may require some workflow and process changes. You’ll receive several notifications at the email address on file both before and after your attestation is due. Your attestation due date will also show up in the TrackIt feature of the UnitedHealthcare Provider Portal.

We may also contact you by phone to review your information. If we’re unable to reach you, we’ll leave a message and a call-back number. It’s important you return our call as soon as possible.

How to verify and attest

If we remove you from the directory, you can be reinstated by completing your data review and attestation using one of the following standard attestation methods:
