- Focus on HEDIS® Controlling High Blood Pressure (CBP) measures
- Identify and reach out to patients with uncontrolled BP or gaps in care
- Leverage care team support and documentation best practices to ensure accurate reporting

- Screen and monitor regularly, including encouraging home blood pressure monitoring
- Educate in clear, culturally responsive ways to support understanding and adherence
- Address SDOH barriers by connecting patients to available community and care coordination resources
- Individualize treatment plans based on patient-specific needs and circumstances
- Ensure follow-up to support sustained blood pressure control
