UnitedHealthcare national Gold Card code update

The national Gold Card code list has been updated, effective Jan. 1, 2026. During our program performance review, we identified codes not utilized by our contracted provider groups and have made the necessary updates to remove the codes from the code list.

The UnitedHealthcare national Gold Card program recognizes contracted provider groups who consistently adhere to evidence-based care guidelines. The Gold Card program applies to all UnitedHealthcare commercial plans, UnitedHealthcare Individual Exchange plans*, UnitedHealthcare® Medicare Advantage plans and UnitedHealthcare Community Plans.
