Your 2026 Provider Administrative Guide is ready

The 2026 UnitedHealthcare Care Provider Administrative Guide for Commercial, Individual Exchange and Medicare Advantage plans is now available.

For currently contracted care providers, the new guide is effective April 1, 2026, allowing you 90 days to review the updates.

For care providers contracted on or after Jan. 1, 2026, the new guide is effective Jan. 1, 2026.

New or changed information includes:

• Updates to communications and tools
• Chapter 7:
  • Discharge notification requirements – clarifying existing expectations
  • Facility-to-facility transfers – clarifying existing expectations
  • Radiology and cardiology requirements – removal of prior authorization requirements for several services categories
• Chapter 8: Preferred diabetic supply brands
• Chapter 10: Revised and includes an update to timely filing requirements for corrected claims
• Capitation and/or delegation supplement: Revised
• UHC Complete supplement
• USHEALTH supplement
