Arizona Medicaid: Vaccines for Children renewal required

Health care professionals enrolled in the Arizona Vaccines for Children (VFC) program must renew their enrollment by Aug. 31, 2026. As part of the state requirements, if you fail to re-enroll, you may be inactivated from the VFC program for non-compliance and members under age 18 may be removed from your panel. Please avoid disruption to your practice by completing your re-enrollment today.
