Gold Card updates reduce prior authorization requirements

The national Gold Card code list has been updated, effective April 1, 2026. This means, certain codes no longer require prior authorization and have been removed from the Gold Card list as part of ongoing reduction efforts.

The UnitedHealthcare national Gold Card program recognizes contracted provider groups who consistently adhere to evidence-based care guidelines. The Gold Card program applies to all UnitedHealthcare commercial plans, UnitedHealthcare Individual Exchange plans, UnitedHealthcare^® Medicare Advantage plans and UnitedHealthcare Community Plans.
