National Gold Card list updates

As of May 1, 2026, the national Gold Card code list has been updated. This means, certain codes will no longer require prior authorization and have been removed from the Gold Card list as part of ongoing prior authorization reduction efforts.

The UnitedHealthcare national Gold Card program recognizes contracted provider groups who consistently adhere to evidence-based care guidelines. The Gold Card program applies to all UnitedHealthcare commercial plans, UnitedHealthcare Individual Exchange plans, UnitedHealthcare® Medicare Advantage plans and UnitedHealthcare Community Plans.
