We’ve updated the onboarding process for Home- and Community-Based Services (HCBS) health care providers supporting UnitedHealthcare Community Plan (Medicaid) and long-term services and supports (LTSS) programs to reduce misrouting and delays.
What’s changed
- The request form now aligns to specific HCBS service types
- Requests are routed automatically to the right team
- Instructions are clearer so you don’t have to resubmit
Services include but are not limited to the following:
- Personal care and in-home support services: Includes personal care, attendant care, homemaker, habilitation, respite and private duty nursing
- Case management: Includes supports coordination
- Residential HCBS settings: Includes assisted living, adult foster care and developmentally disabled group homes
- Adult day and community-based services: Includes adult day care, adult day health and medical day services
- Self-direction and core HCBS supports: Includes fiscal intermediary/financial management services, home delivered meals, personal emergency response systems, assistive technology, home and environmental modifications, and transition services
How to request participation
- Go to the Join our network – Ancillary providers and centers page
- Select Home- and Community-Based Services (HCBS)
- Submit the online request and save your confirmation
These steps help ensure your request reaches the correct onboarding team.
What happens after submission
- We’ll review your application and make a decision within 15 business days
- If approved, a representative will contact you regarding next steps
- If declined, you will receive an email or letter advising of the determination
