Ensure uninterrupted patient care: Help Medicaid patients avoid a lapse in coverage

Your UnitedHealthcare Community Plan (Medicaid) patients may need to renew their coverage. Affected members should have received a redetermination packet from Medicaid, notifying them that they need to reapply (this typically happens every 12 months). If a patient misses the renewal date, their coverage may end.

How you can help

Renewing helps ensure these members can continue to receive services through your office. Please remind your patients to complete and return their redetermination packet as soon as possible.

Member resources

Members can visit the UnitedHealthcare Community Plan page to find state-specific information and details on what they need to know and do, and where they can go if they need more information.
