Out-of-network TIN verification

As a health care professional not currently in our network, please follow the steps below to register your tax ID number (TIN) with UnitedHealthcare.

Before starting, review the necessary information you’ll need to get started to help ensure a smooth and efficient registration process.

What you’ll need to get started

  • One Healthcare ID
  • Your organization’s TIN
  • W-9 tax form within the last year (PDF version) 
  • Documentation for proof of primary location address 

1. Sign in

 

 

2. Enter TIN

  • Enter a TIN and click Start Now

 

 

3. Complete W-9 details

  • Click on W-9 Details 
  • Upload your W-9 tax form 
  • Complete all required fields and click Continue
  • Please note: You can select to save as a draft at any point during this registration process. Your draft will be saved at the bottom of the register TIN start page. Drafts will be saved for 9 days where you can choose to edit or remove the saved registration.

 

 

4. Complete practice data

  • Click on Practice Data
  • Register as Individual or Group
  • Select your practice type 
  • Review and complete the licensure questions 
  • Add your National Provider Identification (NPI) number, then click Continue
    • If you receive an "Unable to Verify NPI" message, please review your NPI information for errors and try again. You can also choose to continue with your registration process by clicking Yes on the message screen.

5. Complete location information

  • Add the location and proof of your primary servicing address, primary billing address and primary remittance address
  • You may add a secondary remittance address, if applicable 
  • Examples of proof of address include utility bills, water bills or phone bills with the address clearly shown on the document 

 

Please note

Include all servicing addresses to ensure there are no delays in processing.  You can add up to 5 servicing addresses, if applicable. After you submit, you will be taken to a confirmation page.

6. Attestation

  • Review W-9, practice data and location(s) 
  • Complete the requested attestation information and submit or save as a draft (draft will be saved for 5 days) 

 

 

7. Confirmation

  • You will receive a confirmation email that your verification information has been received 
  • Please allow 5-7 business days for processing 
  • To check status: 

 

 

8. Submit a claim (if not previously submitted)

  • Submit through EDI or per instructions on the back of the member’s ID card
